Jennifer Luzentales, FNP-C, is a nurse practitioner at Vista Physician Group’s Waukegan office. Her philosophy is that each day is an opportunity to positively impact a patient’s life. “My goal is to educate patients on how to change their lives to become healthy, make informed decisions and achieve their goals,” she says. “I enjoy developing relationships with my patients based on mutual trust to promote health and well-being.”

As a certified family medicine Nurse Practitioner, Jennifer can care for patients of all ages with ailments ranging from pediatric medicine, preventative medicine and women’s health to everyday coughs, colds and allergies.

Jennifer grew up in a rural area of the Philippines where she saw first-hand the importance of adequate access to healthcare services. That’s one reason why she chooses to care for patients in Northern Illinois which includes various rural areas.
